AUTHORITY CONFIRMATION AGREEMENT FOR CLUB AMBASSADOR SIGNUP

This Agreement (the "Agreement") is entered into by and between PaidtoSwitch.ie (the "Company") and the undersigned individual completing this form (the "Signatory"), who is applying to become a Club Ambassador on behalf of the football club identified below (the "Club").

‍1. Representation of Authority
‍By completing and submitting this form, the Signatory confirms and warrants that:

They have been granted explicit authority by the Club to act as a Club Ambassador for the purposes of participating in the "Fuel Your Club" campaign.

They hold a position or have received formal authorization from the Club’s executive committee, governing body, or an individual with decision-making authority (e.g., Chairperson, Secretary, or equivalent).

They understand that they are representing the Club in this campaign and will act in accordance with the Club’s interests and any guidelines provided by the Club.

‍2. Acknowledgment of Responsibility
‍The Signatory understands that:

Submitting this form confirms their role as a Club Ambassador and establishes a binding agreement between the Club and PaidtoSwitch.ie for participation in the "Fuel Your Club" campaign.

Any misrepresentation of authority may result in the termination of the Club’s participation in the campaign and/or other actions as deemed appropriate by PaidtoSwitch.ie.

They are responsible for acting in good faith on behalf of the Club and ensuring compliance with the campaign’s terms.

3. Verification
‍PaidtoSwitch.ie reserves the right to verify the Signatory’s authority through reasonable means, including but not limited to:

Requesting written confirmation from a Club official (e.g., Chairperson, Secretary, or other authorized representative).

Contacting the Club directly to confirm the Signatory’s authorization.

‍5. Acceptance
‍By submitting this form, the Signatory confirms and agrees that:

They have read and understood this Agreement.

They have been expressly authorized by the Club to act as a Club Ambassador.
